The Centers for Medicare and Medicaid Services (CMS) and the Health Resources and Services Administration (HRSA) announced on June 6 an application process for participation in the Federally Qualified Health Center Advanced Primary Care Practice (FQHC APCP) demonstration project. The three-year project is authorized under section 3021 of the Affordable Care Act (ACA). Each FQHC participating in the project will serve as a patient-centered medical home using electronic health records to improve quality of care and to reduce duplication of services. The participating FQHCs will be paid a monthly case management fee for each Medicare beneficiary under a care coordination model endorsed by the National Committee for Quality Assurance (NCQA). Medicare beneficiaries may include individuals with dual Medicare/Medicaid eligibility. The project is expected to pay $42 million in case management fees to approximately 500 FQHCs to coordinate care for 195,000 Medicare beneficiaries. Applications to participate in the project are due August 12.
