The Rhode Island Department of Human Services (DHS) has become the second Medicaid agency in the nation to win approval from the Centers for Medicare and Medicaid Services (CMS) of Medicaid plan amendments under which the state will qualify for 90 percent federal financial participation (FFP) on “health home” services under section 2703 of the Affordable Care Act (ACA). The enhanced federal matching rate will be available for two years (representing approximately $28.8 million). Rhode Island has created health home models designed to improve screening, care planning, and care coordination for adults with severe and persistent mental illness (SPMI) and children and youth with special health care needs (CYSHCN), in order to improve health outcomes, reduce emergency room utilization, and avoid costly inpatient admissions.  Care coordination will be achieved through community mental health centers, family centers for children and youth with special health needs, and other community-based clinics and practitioners.
